To order an official transcript from The Art Institute of Colorado, download the form below and fill it out completely - including student signature. You may fax the form to 1.303.824.2185 and mail in the fee, or mail the completed form with fee to: Registrar's Office, Attn: Transcripts, 1200 Lincoln Street, Denver, CO 80203-2172. There is a $5.00 fee for each transcript ordered. Checks should be made out to The Art Institute of Colorado. Please provide the complete address of each recipient and allow one week for processing your request. Emailed requests cannot be accepted.
